Which documents and test results to bring to a doctor's appointment

Bring your ID and the insurance documents your clinic asks for. From your medical records, the most useful are test results from the past year, hospital discharge summaries, reports from other specialists and scan results.

Write a separate list of the medicines you take, with doses, and any allergies. Put the papers in date order: it is easier for the doctor to see how your values changed when they are in sequence.
